1. Call 1-800-333-9942 or place your order online. We will either mail or fax to you a Customer Blood Test Authorization Form that needs to be signed and returned to us via mail or fax. (NY, NJ and RI residents see disclaimer at end of this page)
2. You will then recieve, via mail or fax, a Blood Requistition Form that you will take with you to one of the LabCorp draw stations closest to you to have your blood drawn. (You will receive a list of the 3 labs closest to you via mail or fax). It's that simple.
3. Blood test results may take up to 5-7 days to be completed. Results will be mailed or faxed upon completion of all tests.
Please note that our retail prices are lower than what many laboratories and doctors charge.
NY, NJ and RI residents will receive a blood draw kit due to state law (additional local draw fee may be incurred). After your blood is drawn, the test results will be mailed to you.
